Referrals and Consultations

All patients seen in our office are seen through an appropriate referral source. If you feel that you have a neurosurgical problem, please request referral from the physician you are currently seeing. Although we do consultations in the office and hospital, they are also done at the request of your physician.

If you need more information about how this system works or have a question about getting a referral from your physician, please feel free to contact our office.

Please note online forms are available to complete and submit securely using our "Online Forms" section of this site. These forms are also available for download in our dedicated "Downloads" section of this site. Please, save these files to your computer, complete, and return downloaded forms by U.S. Postal Service or Fax.  See contact information below for more details.  Note that using the online version of the form(s) will assist us in expediting your request for services.
